DMS Application Checklist

1. Did I apply to Weatherford College and receive a student ID?

2. Did I complete all prerequisite courses?

  • BIOL 2401: Anatomy and Physiology I [must be taken within the past 5 years at the time of admission]
  • BIOL 2402: Anatomy and Physiology II [must be taken within the past 5 years at the time of admission]
  • MATH 1314: College Algebra or higher
  • ENGL 1301: English Composition I
  • PSYC 2301: Introduction to Psychology
  • PHYS X4XX: One of the following: PHYS 1401, PHYS 1415, PHYS 1417 OR PHYS 2425
  • Elective: Language/Philosophy/Culture/Creative Art
  • DMSO 1210: Introduction to Sonography 

3. Did I take the HESI-A2 Entrance Exam and pass reading and math with a 75 or greater?

4. Did I submit proof I started the Hepatitis B Vaccine Series or have a positive titer?

5.Did I complete the Health and Human Sciences Application and mail or email all required documentation listed below?

  • Official or unofficial transcripts from all colleges attended:
    • Transcripts sent to another department cannot be accessed. You must include your transcript(s) in your application.
    • Transcripts should demonstrate prerequisite courses are complete
    • Transcripts should demonstrate the date a degree was earned (if applicable)
  • HESI-A2 scores
  • Proof of Hepatitis B immunization or positive titer
  • Employer letter for medical work experience credit (if applicable)

Failure to submit evidence that all requirements were met will result in denial of application.
